Vitiligo in Children and Its Treatment

Vitiligo is an autoimmune disease that is also seen in children and is characterized by white spots on the skin due to a lack of melanin pigment. According to recent studies in dermatology, there are several important developments and approaches to the treatment of vitiligo in children:

Topical Treatments

Corticosteroids: They continue to be widely used in cases of mild to moderate vitiligo. However, long-term use may cause side effects such as skin thinning, so they should be used with caution.

Calcineurin inhibitors: Calcineurin inhibitors, such as topical tacrolimus and pimecrolimus, are used primarily on the face and thin skin areas. They may be as effective as corticosteroids and have fewer side effects.

Phototherapy

Narrow Band UVB (NB-UVB): This is the most commonly used phototherapy method in children with vitiligo. It is applied 2-3 times a week and can help restore pigmentation in the skin. It is considered a safe and effective treatment option for children.

Excimer Laser: It is applied only to the affected areas by providing targeted treatment. Since it is applied only to the lesion, it has almost no side effects. It is not carcinogenic. It may require shorter treatment and can be effective in small lesions. Treatment responses are very high especially in diseases in the facial area. Treatment is planned for 2-3 sessions per week.

Systemic Treatments

Oral corticosteroids: Can be used for short periods of time to slow down the progression of active, widespread vitiligo.

Janus Kinase (JAK) Inhibitors: Recent studies have shown promising results in vitiligo with JAK inhibitors such as tofacitinib and ruxolitinib, especially in children. However, these drugs are not yet routinely used in children and long-term safety studies are ongoing.

New Biological Therapies

Opzelura (Ruxolitinib Cream): The first topical JAK inhibitor approved for pediatric vitiligo in the U.S. It can be used in children ages 12 and older. Clinical studies have shown effective repigmentation.

Supportive Approaches

Psychological Support: Vitiligo can cause psychological stress and social isolation in children. Therefore, psychological support and counseling are an important part of the treatment.

Nutrition and Supplements: Some studies have shown that supplements such as selenium, vitamin E, vitamin D and B12   shows that it can support the treatment of vitiligo.

In conclusion, vitiligo treatment should be individualized, taking into account the extent of the disease, age, and psychological status of the patient.
